Rather than focusing on curing illness, hospice focuses on quality of life – easing pain, offering support, and helping each patient live fully and peacefully.

We work to manage symptoms, provide emotional and spiritual care, and make sure that patients and their loved ones never feel alone on this journey.

Each hospice agency has its own unique approach, but the heart of the philosophy is the same everywhere: caring for the whole person – body, mind, and spirit – while supporting their loved ones too.

The Importance of Pain Management

One of the most vital parts of hospice care is ensuring that no patient has to experience unnecessary pain or distress.

At Anchor of Hope, our goal is simple: to keep every patient as comfortable and peaceful as possible, every step of the way.

Our nurses work closely with the attending physician and clinical director to develop personalized pain management plans. These may include medication, comfort measures, and non-medical supports like gentle repositioning, soft music, or the calm presence of someone who cares.

We also recognize that pain isn’t only physical – it can be emotional, mental, or spiritual. That’s why our team takes a holistic approach, addressing every layer of discomfort with compassion and sensitivity.

Families often share that once pain and symptoms are well-managed, their loved ones can relax, reconnect, and find peace – creating space for meaningful moments and cherished goodbyes.

A Holistic, Team-Based Approach

Hospice care isn’t something one person can do alone – it takes a team of compassionate professionals and volunteers, all working together to support the patient and their family.

Here’s what that looks like at Anchor of Hope:

Attending Physician (or Medical Director): Oversees each patient’s plan of care, ensuring that medical needs are met with compassion and expertise.

Clinical Director: Provides day-to-day guidance and support for the care team, ensuring care plans are followed closely and every patient receives consistent, thoughtful care.

Registered Nurse (RN) Case Manager: Typically visits patients every 3 to4 days to assess symptoms, manage medications, and coordinate updates with the team.

Certified Nursing Assistant (CNA): The heartbeat of our daily care. Our CNAs visit patients Monday through Friday, helping with bathing, dressing, and comfort – always with a smile and a gentle touch.

Social Worker: Typically visits twice a month (or more as needed) to provide emotional support, help families navigate resources, and be a steady, compassionate presence.

Chaplain: Typically visits twice a month or by request, offering spiritual care, encouragement, and peace – meeting patients wherever they are in their faith journey.

Volunteer Coordinator: Meets with patients and families to determine how a volunteer can best support their needs and desires. Connects and supports our amazing volunteers, ensuring every match is meaningful and every volunteer feels prepared.

Volunteers: Visit weekly or biweekly to offer companionship, conversation, music, or quiet presence. Volunteers bring a special kind of comfort – reminding our patients that they are seen, valued, and loved.

Together, this interdisciplinary team works in harmony – adjusting care plans as needs change and ensuring that every patient is surrounded by care that honors their wishes and comforts their heart.
